At Agape Health we strive on providing and managing supportive services for the senior and disabled population. We believe in providing quality resources to enable our clients the ability to lead productive and independent lives. We are skilled in providing resources for our clients to remain in their homes, or secure an environment that is safe and conductive to their well-being.

We understand that caring for a loved one can be extremely stressful and difficult secondary to numerous factors. At Agape Health we pride ourselves in providing and finding the right independent services to provide relief to family members. Agape Health also provides Case Management and Monitoring in the event that your loved one has to seek alternative placement.

Many of the services that we offer are in-home; however in the event of the need for long-term care placement we also provide the following services:

• Finding service options
• Assessing benefits to find all services that are covered
• Coordinating and distinguishing services from other services
• Planned outings and activities
• Monitoring services
• Options for Adult Day Care
• Options for Respite Services
• Researching and providing options of Long-term Care facilities
• Conducting tours of facilities with family members
• Post site visits after placement
• Annual review of charts (Must be requested by family)
• Attend care-planning meetings for family members
